Tournament Roster: List players in alphabetical order. Use official Babe Ruth League Form. Be sure to have your league President sign the roster. Download Ripken Div Roster - Download Babe Ruth Div Roster
Letter of Tournament Eligibility: Your District Commissioner will provide this.
Players Birth Documents: Babe Ruth Player ID cards preferred. Will also accept copies of Birth Certificate or Passport
Medical Release Form - Download Official Form
Copy of League Accident & Liability Insurance Policy
Proof of Coaching Certification - for each coach - read more
Team Photo - digital photo preferred or 8 x 10 printed photo. Must provide legend identifying each person in photo. Rostered players and coaches ONLY
